Initial fall risk evaluation

Includes:

  • Home safety assessment

    • Identify potential fall hazards and provide recommendations

  • Strength and flexibility testing

  • Posture and Mobility assessment

  • Validated balance tests for objectivity and tracking progress

  • Identifying other modifiable risk factors

    • EX: high risk medications, incontinence, orthostatic hypotension, risk of vitamin D deficiency, fear of falling, low mood

  • Printed detailed report will be provided with recommendations for the home to reduce fall risk

    • Strategies and guided practice to help overcome the fear of falling, rebuild confidence, and restore independence in daily life.

  • Floor recovery training

    • Training on how to safely get up from a fall, proper body mechanics, and how to move with confidence.

Maintenance and prevention plan

For graduated clients who are LOW fall risk

Includes:

  • Early detection of changes in mobility

  • Updated exercise program for continued strength and flexibility

  • Ongoing education and support to optimize safe mobility and independence

  • Return to community and leisure activities based on individual goals
